The 5 Pillars of Later Life Health | Care Momentum

The framework

The 5 Pillars of Later Life Health™

A shared, evidence-grounded framework that makes prevention deliverable, measurable and provable across a whole service. It's facilitative, not prescriptive: no targets, no verdicts, no "right" shape. A mirror that generates insight, not a measure that issues verdicts. Non-clinical, it sits alongside your existing CQC care plans and works with the staff and rotas you already have.

Notice

Every pillar starts with structured noticing: practical, non-clinical prompts that help carers see early changes during the visits they already make.

Intervene

Evidence-based interventions tailored to what carers notice. Practical protocols your carers can deliver within normal visit times.

Measure

Measurable outcomes that prove the impact of prevention to families, referrers, commissioners, and regulators.

01

Pillar 1

Physical

Mobility, Strength, Balance & Falls Prevention

The Physical pillar focuses on maintaining and improving strength, balance, and mobility to prevent falls, the single biggest cause of hospitalisation in older adults. Through everyday noticing and structured exercise, we help clients stay stronger and more independent for longer.

What Carers Notice
  • Changes in steadiness, balance or the way someone walks
  • Difficulty rising from a chair or managing stairs
  • Near-misses, slips or a recent fall
  • Reduced strength or shorter walking distances
Interventions
  • EverFit strength & balance programme
  • Carer-delivered exercise coaching
  • Progressive mobility protocols
  • Falls prevention strategies
Expected Outcomes
  • Reduced falls and hospitalisations
  • Improved confidence and independence
  • Better mobility and balance scores
  • Delayed progression to higher care needs
02

Pillar 2

Cognitive

Memory, Engagement & Dementia-Aware Practice

The Cognitive pillar helps carers notice cognitive changes early and respond with structured stimulation activities. Rather than simply reacting to a dementia diagnosis, we help carers spot changes early and act when it makes the biggest difference.

What Carers Notice
  • Repeating questions or losing the thread of a conversation
  • Missed appointments, medication or familiar routines
  • New confusion, disorientation or word-finding difficulty
  • Changes family members mention between visits
Interventions
  • CogStim24-style structured stimulation
  • Reminiscence and life story work
  • Meaningful activity programming
  • Cognitive exercise routines
Expected Outcomes
  • Earlier identification of cognitive changes
  • Maintained cognitive function through stimulation
  • Better dementia support pathways
  • Reduced family anxiety through proactive monitoring
03

Pillar 3

Nutritional

Diet, Hydration & Malnutrition Prevention

The Nutritional pillar addresses the widespread problem of malnutrition and dehydration in older adults, conditions that accelerate decline but are highly preventable. We help carers notice the early signs and apply practical interventions that work within the real constraints of home care visits.

What Carers Notice
  • Loss of appetite or meals left unfinished
  • Clothes or rings becoming loose, a sign of weight loss
  • Dry mouth, tiredness or confusion that can signal dehydration
  • Out-of-date food or a fridge unchanged between visits
Interventions
  • Fortified plate protocol
  • Hydration monitoring and prompting
  • Texture-modified meal planning
  • Food-first enrichment strategies
Expected Outcomes
  • Improved nutritional status
  • Reduced dehydration-related incidents
  • Better wound healing and recovery
  • Maintained healthy weight and muscle mass
04

Pillar 4

Social & Emotional

Connection, Mood, Purpose & Family Contact

The Social & Emotional pillar tackles loneliness and isolation, major drivers of decline in later life. We help carers build genuine connection and support emotional wellbeing as a core part of every visit.

What Carers Notice
  • Withdrawal from activities once enjoyed
  • Low mood, anxiety or a flatter manner than usual
  • Fewer visitors or less contact with family
  • Expressions of loneliness or a lost sense of purpose
Interventions
  • Meaningful conversation and companionship
  • Community and family connection support
  • Interest- and hobby-based activities
  • Emotional wellbeing check-ins
Expected Outcomes
  • Reduced loneliness and isolation
  • Improved mood and emotional wellbeing
  • Stronger social connections
  • Greater sense of purpose and engagement
05

Pillar 5

Home Safety

Hazards, Equipment & Assistive Technology

The Home Safety pillar addresses the environment where care happens. Many falls and incidents are caused by hazards that are easy to spot and fix: loose rugs, poor lighting, cluttered walkways. We help carers notice these hazards and make or recommend modifications.

What Carers Notice
  • Loose rugs, trailing cables or clutter on walkways
  • Poor lighting or switches that are hard to reach
  • Difficulty using the stairs, bathroom or kitchen safely
  • Aids or equipment unused, broken or no longer suitable
Interventions
  • Carer-led environmental modifications
  • Equipment and aid recommendations
  • Lighting and contrast improvements
  • Assistive technology guidance
Expected Outcomes
  • Safer living environment
  • Reduced in-home falls and accidents
  • Better equipment and aid utilisation
  • Greater confidence for clients and families
